    The July estimate from The Senior Citizens League put the 2027 Social Security cost-of-living adjustment (COLA) at roughly 3.8%. That would be a welcome step up from the 2.8% raise in 2026. Retirees still should not budget the full amount yet. Neither the COLA nor next year’s Medicare premiums are final, and the headline increase is not the amount that reaches the bank account.

    Consider a retired worker collecting approximately $1,600 a month. A 3.8% COLA would add roughly $61. The Medicare Trustees currently project the standard Part B premium at $209.50 for 2027, up $6.60 from $202.90 in 2026. On that early math, Part B would consume about 11% of the raise. That is much better than 2026, when the premium jumped $17.90. But Part B is only one healthcare cost competing for the COLA.

    Who the Hold-Harmless Rule Actually Protects

    Most beneficiaries whose Part B premium is deducted directly from Social Security are protected by the hold-harmless rule, provided they do not pay an income-related surcharge. The rule generally prevents an increase in the standard Part B premium from reducing their net Social Security payment from one year to the next.

    The protection has important limits. People newly enrolling in Part B during 2027 are not protected from the full premium. Neither are beneficiaries who are not yet receiving Social Security and pay Medicare directly. Higher-income retirees subject to the Income-Related Monthly Adjustment Amount, or IRMAA, are also excluded. Someone who enrolled in Medicare during 2026 is not automatically excluded in 2027. If the person is now receiving Social Security and having Part B premiums deducted from the benefit, hold-harmless protection may apply.

    The rule also covers only the standard Part B premium. It does not limit increases in the Part A hospital deductible, skilled nursing coinsurance, Part D costs, Medigap premiums, or Medicare Advantage copays. In 2026, the Part A inpatient deductible climbed to $1,736, while skilled nursing coinsurance for days 21 through 100 reached $217 per day. Those increases apply in full regardless of what happens to the Social Security deposit.

    The Math Hits Smaller Checks Harder

    The same Medicare increase feels very different depending on the size of the benefit. A retiree collecting $3,200 a month would receive roughly $122 more under a 3.8% COLA. Against that, the projected $6.60 Part B increase is modest. Someone receiving $1,300 would gain only about $49. After the projected Part B increase, approximately $43 remains before accounting for drug-plan changes, supplemental coverage, or additional cost sharing.

    The Part B deductible deserves its own line in the budget. Trustees currently project it will rise from $283 in 2026 to $292 in 2027. That deductible generally must be met before Medicare begins sharing the cost of most Part B services.

    The broader picture is worrying. Medicare transfer receipts rose to $1,301.0 billion in Q1 2026, from $1,172.6 billion a year earlier, driven by enrollment growth and premium hikes. Meanwhile, the household savings rate slipped to 3.9% in Q1 2026 from 5.2% a year earlier, so the cushion to absorb another premium jump has thinned.

    What to Do Before January

    Three moves matter more than celebrating an estimated COLA.

    1. Wait for both official numbers. Social Security will announce the final COLA after Q3 inflation data are available, while CMS typically releases the final Part B premium and deductible later in the fall. Build the January budget from those figures, not the July estimate.

    2. Re-shop drug coverage between October 15 and December 7. Part D formularies, pharmacy networks, deductibles, and premiums can change even when the plan name does not. One unfavorable formulary change can consume more than the entire COLA.

    3. Check the 2025 tax return for IRMAA exposure. That return generally determines 2027 Medicare surcharges. If retirement, work reduction, divorce, or the death of a spouse caused income to fall, Form SSA-44 may support a recalculation. A Roth conversion or voluntary property sale does not qualify by itself.

    The 3.8% estimate reflects the gross raise. The number that matters at the kitchen table is what remains after Medicare and the rest of the healthcare budget have had their turn.
