Every serious workplace injury begins with a moment that could have ended differently. It could be a slipping foot that finds its balance or a forklift stopping inches before a collision. The injury could be started by a falling tool missing someone by inches or a driver braking just in time. Most organizations call these events near misses. Human performance science suggests they are something far more valuable as they are opportunities to learn before someone gets hurt. Frankly put, near misses are free lessons into where your employees could be injured.
Recent research demonstrates just how valuable those opportunities can be. Inagaki, Nagata, Odagami, Adi, and Mori followed 2,755 workers who had experienced and reported a near miss. Employees working for organizations that responded inadequately to those reports experienced significantly more workplace accidents during the following year. Organizations providing no meaningful response experienced even higher accident rates. The message is remarkably clear: Responding to near misses measurably influences future workplace safety.
The surprising question is why organizations and employees sometimes overlook such valuable information. The answer begins inside the brain. Human beings naturally habituate to repeated experiences that do not produce negative consequences. A worker walks past the same exposed hazard every day without incident. A shortcut saves time repeatedly. Equipment operates despite a minor malfunction. Gradually, the brain begins treating those conditions as normal because nothing bad has happened. The hazard has not disappeared. Familiarity has simply changed how it is perceived.
Behavioral researchers Robin Dillon-Merrill and Catherine Tinsley described another powerful phenomenon known as near-miss bias. Rather than interpreting a close call as a warning, people often interpret it as evidence that the risk was manageable. “That was close” quietly becomes “Everything turned out fine.” Outcome replaces process. Luck begins masquerading as good judgment. The brain remembers success while overlooking how narrowly failure was avoided.
That pattern should sound familiar. Throughout this series, we have explored optimism bias, automaticity, normalization of deviance, and situational awareness. Near misses bring every one of those concepts together. They reveal the gap between what almost happened and what actually happened. The only difference is that one ended with an injury while the other ended with another opportunity to learn.
Behavioral science also helps explain why many near misses are never reported. Employees may assume someone else will mention it, a pattern consistent with the bystander effect. Others worry they will be blamed, fear creating more work for coworkers, or question whether reporting will make any difference. Amy Edmondson’s research on psychological safety has consistently shown people are far more likely to speak up when they believe their concerns will be heard with curiosity rather than criticism. The quality of an organization’s response often determines whether the next near miss is reported at all.
For many years, workplace safety conversations centered on Heinrich’s Triangle, the idea that reducing minor incidents would naturally reduce serious injuries. Modern research has refined that understanding. Studies examining Serious Injury and Fatality (SIF) events suggest the conditions leading to catastrophic injuries are not always the same as those producing minor injuries. The greatest value of a near miss is not adding another number to a chart. It is identifying events with the potential to produce life-changing consequences and understanding why they almost happened.
Workers’ compensation professionals recognize near misses as one of the most valuable leading indicators available. Every injury avoided protects an employee while also reducing claim frequency, experience modification, operational disruption, and the financial costs associated with medical care, lost productivity, and replacement staffing. More importantly, near misses reveal weaknesses while organizations still have an opportunity to strengthen their systems before someone is harmed. Prevention is always less costly than recovery.
The strongest near-miss programs make reporting simple, timely, and worthwhile. Employees should be able to report hazards quickly without fear of retaliation. Leaders should investigate with the goal of understanding rather than assigning blame. Corrective actions should focus first on eliminating hazards, improving engineering controls, strengthening work processes, and supporting human performance rather than relying solely on reminders to “be more careful.” The most effective organizations close the loop by showing employees how their observations led to meaningful improvements. Learning becomes visible.
Technology is also changing how organizations identify close calls. Computer vision systems, wearable technologies, proximity sensors, and predictive analytics can now detect many unsafe conditions before employees recognize them. These tools create exciting opportunities, although they should strengthen—not replace—the observations, judgment, and communication of the people performing the work. Technology becomes most valuable when it helps organizations learn faster and respond earlier.
Workplace safety is built one decision at a time. Every close call represents a choice. An organization can dismiss it as good luck, or it can treat it as valuable information about how people, equipment, and systems interact. The strongest safety cultures understand learning should never begin with an injury. It should begin with the moments that almost became one.
